He's possessed with that thought of how he would kill Rhys, because for now, that's all he can focus on.
Though any rational thought Jack had stops almost immediately when that scalpel sinks into his eye. He yells out loud and harsh, because it's painful enough that it's immediately dizzying, and he scrapes at the floor to instinctively try and get away.
